What an endoscopy report format should include
Whatever the procedure, a report should let another clinician see what was examined, how well it was seen, what was found and done, and what happens next. Every format above is built around these headings:
- Patient and procedure details. Name, age and sex, UHID or MRN, date and time, referring doctor, endoscopist, scope used.
- Indication. Why the procedure was done and the relevant history.
- Consent, sedation and preparation. Consent, sedation or anaesthesia given, and mucosal visibility or bowel-preparation score.
- Extent and quality indicators. How far the scope went, landmarks reached, and inspection or withdrawal time.
- Findings, region by region. For upper GI endoscopy: oesophagus, stomach, duodenum. For colonoscopy: rectum, each colonic segment, terminal ileum.
- Interventions and specimens. Biopsies, polypectomy, banding, dilatation, and what was sent to the lab.
- Complications. Including a clear statement when there were none.
- Impression and plan. The diagnosis, follow-up, surveillance interval and who acts on the histology.
- Images. Landmark photographs and a photo of every abnormal finding.
- Endoscopist sign-off. Name, registration number, signature, date and time.
Sample endoscopy report (upper GI, normal study)
Here is how the upper GI endoscopy format reads once it is filled in. Patient details are left out.
- Indication
- Dyspepsia for three months, not responding to a proton-pump inhibitor.
- Consent, sedation & visibility
- Written informed consent obtained. Topical pharyngeal anaesthesia; no intravenous sedation. Mucosal visibility good.
- Extent & inspection time
- Examined to the second part of the duodenum, with retroflexion in the stomach. Inspection time 8 minutes.
- Oesophagus
- The oesophagus is normal in calibre with smooth, pink mucosa. No varices, oesophagitis or mass is seen. The gastro-oesophageal junction is at the expected anatomical level.
- Stomach
- The stomach expands well with insufflation. Fundus, body and antral mucosa appear healthy. No ulcer, polyp or active bleeding is seen.
- Duodenum
- The duodenal bulb and second part appear normal. Villous architecture and vascularity are preserved.
- Interventions & specimens
- Antral and body biopsies taken for H. pylori (updated Sydney protocol).
- Complications
- None.
- Impression & plan
- Normal upper GI endoscopy. Review with biopsy results; continue current treatment meanwhile.

Word or PDF format vs. reporting software
A Word or PDF format is fine if you write a few reports a month. Once you are doing several lists a week, retyping findings and pasting in images takes most of the time.
| PDF / Word format | MedPrax Report | |
|---|---|---|
| Cost | Free | Free for 5 reports a month. Paid plans from ₹999 / $19 for 30 days, no automatic renewal |
| Writing findings | Typed or handwritten from scratch for every patient | Tap a common finding, then edit, type or dictate your own wording |
| Endoscopic images | Print and paste, or insert into Word one by one | Capture frames from a scope connected to your laptop, or upload, then pick up to ten |
| Returning patients | Details re-entered every visit | Pick the patient and their details fill in |
| Final report | A Word file or a paper copy | Signed PDF with your name, qualifications and clinic details in the header |
| Corrections | Overwrite the file or rewrite the page | Signed reports stay intact; corrections are added as amendments |
| Sharing with the patient | Print, scan, or send the file | Seven-day link protected by an access code, which you can revoke |

Create free accountFrequently asked questions
Is this endoscopy report format free to download?
Yes. Every PDF and Word format on this page is free to download, print and edit. You do not need an account.
Can I edit the Word format and add my hospital logo?
Yes. The .docx files open in Microsoft Word, Google Docs and LibreOffice. Replace the first line with your hospital or clinic name and logo, and change any section to suit your unit.
What is the difference between the PDF and the Word format?
The PDF is ready to print and fill in by hand. The Word file has typing boxes under each heading, so you can fill it in on a computer and keep your own version.
What should an upper GI endoscopy report format include?
Patient details, indication, consent and sedation, mucosal visibility, extent examined and inspection time, findings for the oesophagus, stomach and duodenum, interventions and specimens, complications, the impression and plan, landmark photographs and the endoscopist’s signature.
Is there a colonoscopy report format?
Yes. The colonoscopy format adds bowel-preparation score, caecal landmarks, withdrawal time and a photo checklist for the rectum, caecum, ileo-caecal valve and every polyp.
Are OGD, EGD and upper GI endoscopy report formats the same?
Yes. Oesophagogastroduodenoscopy (OGD), esophagogastroduodenoscopy (EGD) and upper GI endoscopy are the same examination, so one format covers all three.
Can I fill in the format online instead?
Yes. Each format is also available as a template in MedPrax Report, where you can tap common findings, add scope images and download a signed PDF. The first 5 reports each month are free.
